 Types of Breast Reconstruction  

 

1. Oncoplastic Reconstruction  

 

Oncoplastic reconstruction combines cancer surgery with reconstructive techniques to restore breast shape and symmetry following breast-conserving surgery, such as a lumpectomy. Integrating oncological and plastic surgery principles provides both functional and aesthetic outcomes.

 

 

Volume Displacement  

Volume displacement techniques involve reshaping the remaining breast tissue after removing the tumour. This is ideal for women with moderate to larger breast sizes. The remaining tissue is mobilised and rearranged to fill the defect, preserving breast contour and minimising visible deformities. These techniques often incorporate breast reduction or mastopexy principles, ensuring a natural and symmetrical result.

 

 

Volume Replacement  

Volume replacement involves using tissue from other parts of the body to fill the defect left after tumour removal. This is often recommended for women with smaller breasts or when a significant amount of breast tissue has been removed. Local or distant tissue flaps restore breast volume while maintaining symmetry and contour.

 

Perforator Flaps  

Perforator flaps, such as LICAP, TDAP, and AICAP, are an advanced form of tissue reconstruction in which skin and fat are transferred to the breast while preserving the underlying muscles. These flaps use tissue from the chest wall or back to reconstruct specific breast defects. These techniques offer natural results while minimising the impact on surrounding muscles.

 

These advanced flap techniques offer natural, durable results with fewer long-term complications, making them an excellent choice for oncoplastic reconstruction.  

 

Oncoplastic reconstruction, whether through volume displacement or volume replacement, ensures that breast cancer treatment not only removes the tumour but also focuses on restoring confidence, symmetry, and quality of life. 

 

2. Implant-Based Reconstruction  

Implant-based reconstruction involves using silicone or saline implants to restore breast shape. This option is often suitable when there is sufficient skin and tissue to support the implant or following skin expansion procedures.  

The advantages of this technique include less morbidity, a shorter recovery period, and the ability for patients to return to their everyday routines as quickly as possible.

 

3. Autologous Reconstruction  

Autologous reconstruction uses tissue from another body part, such as the abdomen, back, or thighs, to recreate the breast.  

 

The primary benefits of this approach are its durability and superior long-term aesthetic outcomes. However, it is a more complex procedure requiring longer recovery and is typically performed by a specialised team, including plastic surgeons.  

  

 Timing of Reconstruction  

 

Immediate reconstruction is performed at the time of mastectomy, offering the advantage of combining surgeries and reducing recovery time. Delayed reconstruction is performed months or years after mastectomy, often after other treatments such as radiation or chemotherapy.  

 

The choice between immediate and delayed reconstruction depends on individual treatment plans, medical conditions, and patient preferences.
